How To Make A Request:
Under the Freedom of Information Act, 2014, you are entitled to apply for access to information not otherwise publicly available.
- The right for each person to access information held by public bodies.
- The right for each person to have official information relating to him/herself amended where it is incomplete, incorrect or misleading.
- The right to be given reasons for decisions taken by public bodies that affect them. This right means that from the 21st April, 1998 people can seek access to personal information held on them no matter when the information was created, and to other records created after 21 April, 1998.

Patient signed consent and a copy of photo identification must be provided with each request.
